Home Health Care Services

  • Caregivers are available for referral on an hourly or live-in basis for services as described below.
  • CHN accepts private payment as well as assignment of benefits for long term care insurance polices.  We process claims from start to finish.
Custodial (Personal) Care Household Activities

Caregivers are trained to assist with the following Activities of Daily Living:

  • Bathing
  • Dressing
  • Feeding
  • Toileting
  • Transfers (i.e. getting safely into and out of a bed, chair, wheelchair, car)

Caregivers are trained to assist in the following areas:

  • Light Housekeeping
  • Laundry
  • Meal Preparation
  • Grocery Shopping
  • Transportation to doctor visits and other appointments
  • Medication Reminders
